To be featured simply fill out the online form below, OR download and print the form to the right of the online form and mail it with a photo to our office (mailing address is on the printable form) OR visit our office at the same address and we will be happy to help. Photos received via mail will be scanned and returned to sender. Your provided information will be turned into a slide in the format featured to the left. We suggest that active law enforcement officers are submitted as a department, instead of as individuals. We encourage a donation with every honoree submission.
